About the title
A new patient is admitted to psychotherapist Rose, who works at a facility for the mentally ill. A young, promising woman with a doctoral degree insists she is not crazy: she is simply being stalked by a demon that takes the form of a person with a terrifying smile and whispers in her ear about her impending death. No matter how hard Rose tries to calm the patient, after a hysterical fit the woman smiles ominously and slits her own throat. After the incident, the smiling demon begins to stalk Rose herself.
